 Placing a family member in a long-term care facility can be a difficult decision to make.
Naturally, families want their loved one to be cared for as they would do if able. St. Barnabas Social Services can help families with this transition by providing a wide variety of services:
- Explain the contract agreement so families understand the contents

Set up power of attorney for living will, medical and/or financial POA

Provide in-house notary public at no charge to families or residents

Conduct social histories as part of each resident's assessment. This assures continuity of care, builds relationships between staff and family, and helps staff to really know the patient/resident and their preferences and habits.

Provide emotional support for residents with no family, in essence becoming their family

Make regular room visits to each resident/patient twice weekly, in addition to scheduled visits

Arrange for family member to stay with resident if in a private room, or in a semi-private room with no roommate
- Arrange care plan meetings at admission and at quarterly intervals thereafter
- Assist with Medicaid applications, insurance updates, or any like assistance
- Arrange transportation
- Set up appointments. The Social Services Department works very closely with the Quality of Life Department as regards scheduling.
- Order and/or coordinate, on discharge, any medical equipment or home health services needed
- Provide resources for additional outside assistance
- Advocate for resident/patient rights
- Intervene as necessary to help staff and families focus on what's best for the patient or resident
- The Social Services Department has an open door policy - high availability with no appointment necessary
